Paracetamol, alcohol, caffeine and heroin are all, in the strict biological sense, drugs: substances that alter how the body or the nervous system functions once they are absorbed into the blood. What separates them is not whether they act on the brain, but how they act, how strong that action is, and what legal controls exist around their use.

This topic explains what a drug is and the difference between legal and illegal drugs, the specific dangers of heroin, cannabis and cocaine, and how paracetamol acts on the nervous system. It then covers the effects of alcohol on the nervous system and liver, before turning to the biology of two common mental illnesses, schizophrenia and depression, and three conditions that progressively affect brain function: Alzheimer's disease, vascular dementia and Parkinson's disease.

A drug is any substance, other than food, that alters the way the body or mind works once it has been taken into the body. Many drugs act on the nervous system directly, changing how neurones transmit or respond to signals. Drugs are divided by law into legal drugs, which can be manufactured, sold and used within rules set by government, sometimes with age restrictions (examples include paracetamol, caffeine, nicotine and alcohol), and illegal drugs, whose manufacture, sale or possession is banned outright or tightly restricted to licensed medical use because of the risks they carry (examples include heroin, cannabis and cocaine in most jurisdictions). Legal status is set by law and can differ between countries; it is not, by itself, a measure of how strongly a drug affects the body.
