In 1848 a British surgeon could open a patient without knowing what caused infection, operating in a coat stiff with old blood, and a government could pass a public health act that no town was obliged to obey. A century later penicillin was in mass production and the National Health Service treated anyone who walked through the door, free at the point of use.
