Changes In Medicine, C1848-c1948

Aperçu

In 1848 a British surgeon could open a patient without knowing what caused infection, operating in a coat stiff with old blood, and a government could pass a public health act that no town was obliged to obey. A century later penicillin was in mass production and the National Health Service treated anyone who walked through the door, free at the point of use. The distance travelled in one hundred years is greater than in the previous thousand.

What makes this a breadth study rather than a story is that the progress was uneven. Long stretches produced almost nothing, then a single idea or a single war changed everything at once. Your job is to explain the rhythm: which developments were turning points, what kept holding progress back, and why war repeatedly did in four years what peacetime had failed to do in forty.

Objectifs

  1. Progress in the mid-19th century: barriers to progress, Florence Nightingale and changes in nursing, Simpson and chloroform, Chadwick and the Public Health Act (1848), the cholera threat and the work of Snow
  2. Discovery and development, 1860-75: Pasteur and the germ theory, Lister and antiseptics, the Public Health Act (1875), Nightingale and continuing improvements, Elizabeth Garrett and women in medicine
  3. Accelerating change, 1875-1905: Koch and bacteriology, aseptic surgery, the fight against germs, blood transfusions, magic bullets and the work of Ehrlich, radioactivity and Marie Curie
  4. Government action and war, 1905-20: the measures of the Liberal Governments, the importance of the First World War for women in medicine and improvements in treatment, surgery, x-rays, blood transfusion
  5. Advances in medicine, surgery and public health 1920-48: the development of penicillin, Fleming, Florey and Chain, the importance of the Second World War, Beveridge, the development of the NHS

Note de cours

Four factors drive change across this whole period and they are worth naming before any detail: scientific understanding, chiefly germ theory; technology, from anaesthetics to X-rays to industrial fermentation; government action, which moves from permissive to compulsory to universal; and war, which supplies both urgency and money. Almost every development in the specification can be filed under at least one of the four, and the best answers explain how they interacted rather than treating them as a list.
